Please submit payroll documents safely and securely through the DocuSign link provided below.  BEFORE you click the link be prepared to upload digital images of your ID (US Passport or driver’s license/SS card combination), dental license, and bank account/routing numbers (optional should you opt for direct deposit).  If you prepare this information ahead of time the DocuSign signing process will go quickly.

When you finish the payroll documents it will prompt you to either save or print the documents.  Please print them so you can bring them with you to temp jobs or working interviews.
